BBC chief quits over report on Queen trailer

Two of the most senior figures in British broadcasting have resigned after a report into a misleading trailer for a BBC documentary about the Queen found the corporation guilty of "misjudgments, poor practice and ineffective systems".

Peter Fincham, the controller of BBC1, resigned with immediate effect, as did Stephen Lambert, the chief creative officer of RDF Media, the independent production company that made A Year With The Queen.
